Application for Individuals

You can download the application form from the link below and print it out. Once you print the form, fill it in and have your doctor fill in the appropriate section, then mail or deliver the completed form to the SPARC BC office.

We cannot accept faxed applications, as the original signatures of the applicant and doctor are required before we can process the form.

Application for Organizations

Some organizations that regularly transport people with mobility impairments qualify for a parking permit for people with disabilities that allows them to park in designated accessible parking zones.

Membership in SPARC BC

We also encourage parking permit applicants to become members of SPARC BC in order to further support our charitable research, advocacy, and education efforts.
